Role Overview
Are you passionate about making a real difference in people’s lives? Cambridgeshire County Council is seeking an Adult Support Coordinator to join our dynamic Community Team. You will play a vital role in supporting adults across Cambridge and South Cambridgeshire, helping them achieve independence, wellbeing, and positive outcomes.
As part of a collaborative team, you will work closely with health professionals, social workers, and partner agencies to provide coordinated, person-centred care. This role offers a unique opportunity to contribute to innovative social care practices while building strong relationships within the local community.
Key Responsibilities
1. Assessment & Care Planning
- Conduct Social Care and Mental Capacity Act assessments
- Develop and implement care and support plans tailored to individual needs
- Use evidence-based, proportionate recording with a focus on strengths-based practice
2. Community Liaison & Partnership Working
- Collaborate with multidisciplinary teams including health services, housing, and voluntary organisations
- Act as a point of contact for families, carers, and professionals
- Support adults to access services fairly and effectively, ensuring inclusion
3. Case Management & Coordination
- Manage a caseload of adults requiring community support
- Monitor progress and adjust plans as needed to achieve optimal outcomes
- Advocate for service users and support their engagement with care pathways
4. Service Development & Innovation
- Participate in team development and improvement initiatives
- Embrace new ways of working to enhance flexibility and service delivery
- Contribute to a positive, inclusive, and supportive workplace culture
Person Specification
Essential Requirements
- Genuine desire to support adults and put individuals at the centre of care
- Inquisitive and analytical, able to identify needs and opportunities for support
- Strong interpersonal and relationship-building skills
- Ability to work collaboratively across professional and organisational boundaries
- Commitment to continuous professional development
Desirable
- Previous experience within adult social care or community-based roles
- Familiarity with strength-based approaches and evidence-based interventions
Safeguarding & Background Checks
This position requires an Enhanced Disclosure and Barring Service (DBS) check. Applicants must demonstrate suitability to work with vulnerable adults.
Visa & Eligibility
Applicants requiring sponsorship under the Skilled Worker route may apply; however, Health & Care visa sponsorship is not available for this role. UKVI guidance applies.
