Job Description

Senior Community Mental Health Nurse | Somerset Urgent Care Pathway

  • Location: Somerset (Community-based)
  • Salary: Competitive (NHS Banding)
Rethink Crisis Care. Join a Team That Cares for You Too.

Are you an RMN who thrives in fast-paced environments but values the depth of community-based recovery? We are looking for a compassionate, resilient Senior Mental Health Nurse to join our Home Treatment and First Response Team.

In this role, you aren’t just a clinician you are a lifeline. You will provide the vital intervention needed to help individuals navigate acute crises within the safety and comfort of their own homes, preventing unnecessary hospital admissions and championing person-centered recovery.

The Role: Impact and Responsibilities

Working across a 24-hour flexible rota, you will be a key player in our multidisciplinary team. Your day-to-day will involve:

  • Expert Crisis Response: Conducting holistic psychosocial and risk assessments in homes, hospitals, and community hubs.
  • Creative Care Planning: Designing evidence-based, recovery-focused plans that go beyond the symptoms to treat the person.

  • Therapeutic Intervention: Delivering short-term support, including emotional skills training and psychoeducation.

  • Clinical Leadership: Acting as a “gatekeeper” for inpatient beds, facilitating early discharges, and supervising Band 5 nurses and students.
  • System Navigation: Working as a named keyworker to bridge the gap between families, social care, and medical teams.

Why Somerset? (The “Best of Both” Lifestyle)

We believe your life outside of work is just as important as your career. Working in Somerset offers:

  • The View: Spend your days off exploring Areas of Outstanding Natural Beauty and stunning coastlines.
  • The Connection: Enjoy the peace of the countryside while staying a short hop away from the vibrant city life of Bristol, Bath, and Exeter. London is just two hours by train.

  • The Practicalities: Benefit from high-quality local schools and house prices that remain reasonable compared to much of the South West.

What’s In It For You?

As part of the NHS family, you’ll receive:

  • Wellbeing First: A supportive, close-knit team culture that understands the pressure of crisis work.
  • Financial Security: A market-leading pension and generous annual leave.

  • Perks: Access to Blue Light Card discounts and exclusive NHS savings.
  • Growth: Real pathways for professional development and clinical specialization.

Who You Are

The Essentials:

  • RMN Qualification: Current NMC registration is a must.

  • Experience: You’ve spent significant time in community mental health and feel confident managing acute risks.
  • Mobility: You have the ability to travel across the Trust area to meet patients where they are.

  • Communication: You can talk about complex, distressing issues with sensitivity and clarity.

The Extra Mile (Desirable):

  • Innovation in practice—you like finding new ways to improve patient outcomes.

  • Experience in acute settings or specialized post-qualifying training.

  • A natural ability to lead and delegate within a team.
