Role Overview

Tees Esk and Wear Valleys NHS Foundation Trust is seeking an experienced Advanced Clinical Practitioner (ACP) to join its Community Mental Health Team supporting adults living in the Derwentside, Stanley, and Chester-le-Street areas of County Durham.

This role offers an excellent opportunity for an experienced clinician who is ready to work at an advanced level and contribute to high-quality, patient-centred mental healthcare within the community. The successful candidate will work autonomously while collaborating with a multidisciplinary team to assess, treat, and support individuals experiencing complex mental health needs.

About the Position

As an Advanced Clinical Practitioner, you will function as a senior clinical decision-maker and play a key role in shaping service delivery. The role involves working across the four pillars of advanced practice: clinical care, leadership, education, and research.

You will be responsible for delivering advanced clinical assessments, developing treatment plans, and supporting improvements in mental health services across the community care pathway.

Key Responsibilities

Advanced Clinical Practice

The post holder will provide comprehensive patient care by:

  • Undertaking holistic assessments for individuals with complex mental health needs.
  • Diagnosing conditions and developing evidence-based management plans.
  • Prescribing medications and therapeutic interventions within professional scope of practice.
  • Managing ongoing care and monitoring treatment outcomes.

Leadership and Service Development

The ACP will support service improvement initiatives by:

  • Leading or contributing to quality improvement projects within the service.
  • Supporting the implementation of innovative care models to improve patient outcomes.
  • Influencing service development through collaboration with clinical leaders.

Education and Workforce Development

The role includes mentoring and developing the workforce by:

  • Providing clinical supervision and mentoring to junior colleagues.
  • Supporting the training and assessment of healthcare learners.
  • Promoting best practice and professional development across the multidisciplinary team.

Collaborative Working

The post holder will work closely with professionals across:

  • Primary care
  • Secondary mental health services
  • Community health services
  • Social care organisations
  • This integrated approach ensures patients receive coordinated and personalised care.

Essential Qualifications and Experience

Applicants must meet the following requirements:

  • Current professional registration with the Nursing and Midwifery Council
  • A Master’s degree in Advanced Clinical Practice, or a Master’s degree in a relevant clinical subject with recognised ACP accreditation
  • Significant experience at Band 7 level or equivalent, demonstrating readiness for advanced practice
  • Evidence of teaching, supervision, or mentorship experience within clinical settings
  • Strong understanding of advanced nursing practice and professional scope of responsibility

Desirable Qualifications

Additional qualifications or experience that would strengthen an application include:

  • Teaching or education qualifications
  • Mentoring or coaching certifications
  • Formal training in clinical supervision
  • Experience contributing to research or service development initiatives.
