Make a Meaningful Difference in Community Learning Disability Care
We are seeking an experienced and compassionate Band 6 Learning Disability Nurse to join an established Integrated Learning Disability Team serving County Durham. This is a rewarding opportunity to support adults with learning disabilities and behaviours that challenge, working in close partnership with colleagues across health and social care.
The team operates within a fully integrated structure led by Durham County Council, using the Azeus recording system while maintaining access to NHS clinical systems and secure NHS mail.

The team works collectively to deliver trauma-informed, recovery-focused care in line with statutory frameworks and best practice pathways.
What You’ll Be Responsible For
Clinical Practice
- Manage a complex caseload of adults with learning disabilities.
- Conduct specialist assessments and develop person-centred care plans.
- Deliver and evaluate evidence-based interventions.
- Undertake clinical risk assessments and safeguarding actions where required.
- Apply relevant legal frameworks including the Care Act, Mental Capacity Act and Mental Health Act.
Leadership & Supervision
- Provide clinical supervision and mentorship to junior staff and students.
- Support practice assessors/supervisors and contribute to workforce development.
- Deliver training to colleagues and partner agencies.
Collaborative Working
- Work in partnership with families, carers and multi-agency stakeholders.
- Promote integrated care planning with health and social care services.
- Contribute to service development and quality improvement initiatives.
What We’re Looking For
Essential Registration & Qualifications
- Current NMC registration (RNLD or RNMH).
- Evidence of Continuing Professional Development.
- Recognised Practice Supervisor/Assessor (or willingness to complete within 6 months).
- Clinical supervision training (or willingness to undertake within 6 months).
- Minimum 18 months post-qualification experience, including completion of preceptorship or competency framework.
- Registration requirements can be reviewed via the Nursing and Midwifery Council.
Essential Knowledge & Experience
- Postgraduate-level understanding of evidence-based learning disability practice.
- Safeguarding knowledge and practical application.
- Clinical governance principles.
- Risk assessment and risk management expertise.
- Knowledge of medicines management including injectable therapies.
- Understanding of psychological care models.
- Experience working within multidisciplinary teams.
- Experience supervising students in clinical placements.
Travel & Flexibility
The role requires independent travel across County Durham. Applicants must have access to suitable transport for business use. Reasonable adjustments will be considered in line with the Equality Act 2010.
