Lead Meaningful Change in Integrated Learning Disability Care
An exciting opportunity has arisen for an experienced and motivated Learning Disability Nurse to join a well-established Integrated Learning Disability Team in County Durham.
This role sits within a collaborative partnership between Tees Esk and Wear Valleys NHS Foundation Trust and Durham County Council. Durham County Council acts as the lead authority, and clinical documentation is completed using the Council’s Azeus recording system, alongside access to NHS systems and NHS mail.
You will work within a highly skilled multi-disciplinary team including Social Workers, Psychiatrists, Psychologists, Physiotherapists, Occupational Therapists, Speech and Language Therapists, and administrative colleagues — all committed to improving the lives of adults with learning disabilities.
The Role
Person-Centred, Specialist, Community-Focused
As a Band 6 Senior Learning Disability Nurse, you will:
- Manage a complex caseload of adults with learning disabilities and behaviours that challenge
- Complete comprehensive assessments, develop care plans, and deliver specialist interventions
- Apply trauma-informed and recovery-based approaches
- Implement the Care Act and relevant Clinical Care Pathways
- Monitor, evaluate and adjust interventions based on outcomes
- Provide clinical supervision and mentorship to junior staff and students
- Deliver advice, support and training to colleagues and partner agencies
- Contribute to quality improvement initiatives
You will be professionally accountable for ensuring individuals remain at the centre of their care, actively involved in planning and decision-making.
Service Model & Working Pattern
- Core hours: Monday to Friday, 9am–5pm
- Participation in Nurse On-Call rota (8pm–8am out-of-hours)
- Community-based service across County Durham
- Independent travel required across the locality (reasonable adjustments considered under the Equality Act 2010)
What We’re Looking For
Essential Qualifications & Experience
- Current NMC registration (RNLD, RNMH, or RGN where appropriate)
- Minimum 18 months post-qualification experience (including completed preceptorship/competency framework)
- Evidence of Continuing Professional Development
- Practice Supervisor/Assessor status (or willingness to achieve within 6 months)
- Experience supervising staff or students
- Experience working in multidisciplinary and multi-agency settings
- Involvement in quality improvement initiatives
- NMC registration guidance
Essential Knowledge & Skills
- Strong understanding of evidence-based practice in Learning Disability nursing
- Clinical risk assessment and management expertise
- Safeguarding knowledge and practical application
- Working knowledge of medicines management, including injections
- Understanding of psychological models of care
- Knowledge of research and service development methodologies
