This role offers a unique opportunity to combine general practice with specialist palliative care, directly supporting patients with advanced illness and those nearing end-of-life. You’ll be at the heart of anticipatory care planning, symptom management, and multidisciplinary coordination across the community, care homes, and PCN services.

About the Role

As a GP with a special interest in palliative care, you will:

  • Deliver both generalist and specialist palliative care in practice and community settings
  • Lead on anticipatory care planning, advance care planning, and DNACPR discussions
  • Support symptom management and medication optimisation, aiming to reduce avoidable hospital admissions
  • Act as a clinical lead, mentoring colleagues and contributing to service improvement initiatives
  • Collaborate with community nursing teams, specialist palliative care services, hospices, secondary care, and social care providers
  • This role offers a flexible portfolio approach, with responsibilities potentially spanning multiple services and clinical modalities.

Candidate Profile

We are looking for a GP who:

  • Is registered with the General Medical Council and on the GP Specialty Register
  • Holds a current NHS Performers List registration
  • Has post-CCT experience in primary or urgent care
  • Has postgraduate experience or qualifications in palliative care (e.g., Diploma in Palliative Medicine, RCGP End of Life Care modules)
  • Demonstrates experience in multidisciplinary collaboration and patient-centred care
  • Has excellent organisational skills and a proactive approach to both clinical and operational responsibilities
