Make a Real Impact in Mental Health Care
Somerset NHS Foundation Trust is seeking a dedicated and experienced Senior Mental Health Liaison Worker to join our Community Mental Health Service. This role is ideal for professionals who are passionate about improving mental health outcomes, bridging care between primary and secondary services, and supporting adults experiencing mental or emotional distress, including older adults and individuals with complex needs.
You will play a pivotal role in integrated care pathways, working closely with GPs, multidisciplinary teams, and voluntary sector partners, ensuring that patients receive seamless, compassionate, and effective support.
Key Responsibilities
Clinical Leadership & Assessment
- Lead assessments and interventions for adults (18+) with a wide range of mental health needs.
- Act as the lead professional for your own caseload, coordinating care and developing evidence-based treatment plans.
- Deliver both individual and group interventions, including early intervention for high-risk or complex cases.
Supervision & Training
- Provide clinical supervision, guidance, and mentorship to colleagues across mental health and voluntary sector teams.
- Contribute to ongoing staff training, professional development, and service improvement initiatives.
Collaboration & Partnership
- Build strong relationships with GPs, neighbourhood teams, and community partners to ensure continuity of care.
- Act as an advocate for patients, ensuring their voices are central to care planning.
Service Development & Governance
- Engage in service development, audit, and research to enhance the quality of mental health provision.
- Maintain accurate clinical records, adhere to safeguarding requirements, and uphold professional and ethical standards.
Independent Prescribing
- Practise as a non-medical prescriber, reviewing and optimising medications in line with current professional standards.
Person Specification
Essential Qualifications & Registration
- Registered Nurse or Allied Health Professional
- Independent Non-Medical Prescriber with the relevant professional body
- Commitment to ongoing Continuing Professional Development (CPD) in mental health
Desirable Skills & Experience
- Motivational Interviewing and Relapse Prevention training
- Experience working with clients in the community
- Teaching, training, or mentoring other staff
Experience Requirements
- Significant experience working with adults presenting with complex mental health and emotional needs
- Experience in clinical supervision and staff mentoring
- Experience in delivering evidence-based interventions
