The Opportunity — Community GP Care with Specialist Impact

Minehead Medical Centre is seeking a compassionate GP who brings both strong general practice skills and a focused interest in palliative and end-of-life care. This is a role for a clinician who values continuity, multidisciplinary teamwork, and meaningful patient relationships — particularly supporting people with advanced progressive illness.

You’ll provide holistic care in the surgery and within patients’ homes, helping individuals and families navigate complex clinical and emotional decisions with dignity and clarity. Beyond direct care, you’ll act as a local clinical champion for palliative practice, strengthening collaboration across primary care, community teams, hospices, and social services.

What You’ll Be Doing

  • Deliver comprehensive GP care alongside specialist palliative input
  • Lead anticipatory and advance care planning conversations
  • Support symptom control, medication optimisation, and crisis prevention
  • Coordinate care with district nurses, hospices, and community partners
  • Reduce avoidable hospital admissions through proactive planning
  • Provide clinical leadership and mentorship to colleagues
  • Contribute to service improvement and quality initiatives
  • Maintain accurate documentation and uphold best-practice governance

Who We’re Looking For

Essential

  • GP registered with the General Medical Council
  • Listed on the NHS GP Performers List with post-CCT experience
  • Evidence of postgraduate palliative care training or equivalent experience
  • Strong multidisciplinary working skills
  • Commitment to patient-centred, community-focused care
  • Organised, proactive approach to clinical responsibilities

Desirable

  • Diploma or structured training in palliative medicine
  • Experience leading quality or service improvement initiatives
