We are seeking compassionate and dedicated Support Workers to provide high-quality, person-centred care to individuals within their own homes and in the community. This role involves delivering holistic care while promoting dignity, independence, and equality at all times.
Key Responsibilities:
- Deliver high standards of holistic care, maintaining the dignity, privacy, and equality of each patient or service user.
- Apply the principles of the Fundamentals of Care in daily practice to ensure quality services for patients and clients.
- Ensure the service user’s home is clean and tidy; report any untidiness or clutter to the appropriate person.
- Complete patient menu cards in collaboration with patients, considering dietary needs as identified by a qualified nurse.
- Support protected mealtimes by ensuring meals and beverages are served promptly and safely, hands are washed, and the environment is appropriate for dining.
- Perform patient observations (temperature, pulse, and respiration) following appropriate training and assessment.
- Assist in the safe moving and handling of service users in accordance with organisational policies and guidelines.
- Respond appropriately and promptly to urgent or emergency situations.
- Communicate basic information clearly and respectfully to service users and their carers, upholding confidentiality and dignity.
- Provide personal care and support with domestic tasks, including washing, dressing, bathing, toileting, and medication monitoring or assistance.
- Encourage and support service users to maintain their independence while receiving care, tailored to their individual needs and support plans.
- Carry out a range of personal, social, and enabling care tasks both at home and in the community.
- Communicate effectively with service users and their families to deliver care aligned with their person-centred support plan.
- Perform light domestic duties such as meal preparation, essential cleaning, shopping, bill payments, and fire safety checks.
- Provide enabling support, including supervision, encouragement, and coaching to empower service users to lead independent and fulfilling lives.
- Escort or accompany service users to appointments, community outings, and other social events as required.

- A valid driving licence and prior caregiving experience are desirable but not essential.
