Diabetes Specialist Nurse – West & North Hampshire Community Diabetes Team

About the Role

An exciting opportunity has arisen to join our dynamic West and North Hampshire Community Diabetes Team as a Diabetes Specialist Nurse. This role focuses on delivering high-quality diabetes care in community settings and supporting professional development through targeted diabetes education for healthcare colleagues.

You will work within a multidisciplinary team, offering expert clinical advice and managing patients with complex needs. Your support will extend to GPs and other professionals, particularly in areas such as prescribing and titrating diabetes medications.

This is a fantastic opportunity for an experienced nurse to further develop their clinical and leadership skills in a supportive and innovative environment.

Key Responsibilities

  • Provide high-quality, person-centred care to people with diabetes referred to the service across a defined geographical area.
  • Use advanced clinical skills to assess, treat, and co-create care plans with patients.
  • Act as a point of contact for people with diabetes, offering specialist advice and appropriate referral pathways.
  • Use motivational interviewing techniques and the principles of person-centred care, referencing the “Language Matters” guidance to promote self-care and improved health outcomes.
  • Monitor and support patients with Type 1 and Type 2 diabetes via various platforms—face-to-face clinics, virtual consultations, telephone helplines, and group education sessions.
  • Prescribe and adjust diabetes medications according to local and national clinical guidelines.
  • Deliver structured education programmes both in-person and virtually (e.g. via Zoom).
  • Collaborate with colleagues to plan and coordinate clinics in line with NICE guidelines to ensure timely access and continuity of care.
  • Empower patients to make informed decisions about their treatment, including insulin adjustments.
  • Contribute to the supervision of practice, teaching, clinical audits, and implementation of evidence-based care.

About You

Essential Qualifications and Experience

  • Registered Nurse (NMC Level 1)
  • Post-registration qualification in diabetes care
  • Extended Nurse Prescribing qualification (V300)
  • Proven experience in diabetes care, ideally within a community setting
  • Excellent communication skills and a person-centred approach

Desirable Skills

  • Experience in delivering group education
  • Familiarity with virtual health platforms and digital care delivery
  • Confidence in supporting professional colleagues through training and clinical advice
