JOB SUMMARY
Urgent Community Response.
Band 7 Clinical Nurse Post.
The unplanned urgent community response style of care is well established in Walsall, with a fully integrated domiciliary service and solid links with Social Services and the Acute Sector.
The post holder will be responsible for coordinating care and guiding patients along pathways across the primary and secondary care interface, as well as preventing avoidable hospitalisations through a two-hour national focused response.

An opportunity has arisen to advance your career as an urgent community response nurse, fulfilling the expectations of the New Ambition for Older People-Standard 7 high level of performance metrics that prevent hospital admission and expedite hospital release from front-line service areas.
The position holder will work as part of one of our unscheduled, front-door services, proactively managing a caseload of patients requiring complicated case management in order to avoid hospital admissions and improve patient health outcomes for a limited time.
Working autonomously, the post holder will be responsible for proactively monitoring and managing patients on the caseload, as well as supporting and collaborating with the larger nursing team on the community caseload.
Walsall Healthcare NHS Trust is an integrated trust and the sole provider of NHS acute care in Walsall, servicing a population of 286,700. It provides inpatient and outpatient treatments at the Manor Hospital, as well as a variety of community services. Walsall Manor provides the entire range of district general hospital services under one roof. The £170 million expansion was completed in 2010, and ongoing upgrades guarantee that the Trust now has cutting-edge Critical Care Unit, Neonatal Unit, Obstetric Theatre, and Integrated Assessment Unit capabilities. Work is already well started on the multi-million pound new Urgent and Emergency Care Centre, which will include a new Emergency Department (including a Children’s Emergency Department), a Paediatric Assessment Unit, an Acute Medical Unit, and an Urgent Care Centre. This is anticipated to be completed this year.

You will be a member of a multidisciplinary, multi-agency team with responsibility for a diverse caseload of clients in their own homes around the borough, reporting to the Advanced Clinical Practitioner Lead within the UCR Team. You will primarily collaborate with Older People’s Services to create and support this client group’s ability to remain in their homes through expanded services. You will need to work flexibly to fulfil the service’s demands over a seven-day period and be able to collaborate well with partnership agencies such as acute, WMAS, and GP’s.
