At Pacific Gardens Nursing and Rehabilitation Center, we’re more than a healthcare facility — we’re a community dedicated to restoring independence and dignity to every patient we serve.
We specialize in post-acute rehabilitation, long-term care, and geriatric outpatient therapy, and are currently seeking a passionate Speech-Language Pathologist (SLP) to join our in-house therapy team on a PRN basis.

As part of our Flagship Therapy Network, you’ll have access to industry-leading continuing education and certification programs in areas such as dysphagia management, cognitive rehabilitation, dementia care, and neuro recovery.

 Your Role

As a Speech-Language Pathologist, you will provide personalized therapy to residents experiencing communication, cognitive, or swallowing challenges.
You’ll collaborate with a multidisciplinary team to help patients achieve their highest possible level of function and independence.

Key Responsibilities

  • Conduct comprehensive assessments for speech, language, cognitive, and swallowing disorders.
  • Develop and implement individualized treatment plans following state and federal healthcare regulations.
  • Deliver rehabilitative therapy sessions and monitor progress toward clinical goals.
  • Adjust interventions based on patient response and collaborate with other care providers.
  • Offer education and counseling to patients, caregivers, and families on communication and swallowing management.
  • Prepare accurate documentation and discharge summaries in compliance with professional standards.

Qualifications

Required:

  • Graduate degree in Speech-Language Pathology (ASHA-accredited program)
  • Excellent communication, assessment, and documentation skills

Preferred:

  • Experience in post-acute, geriatric, or long-term care environments
  • Familiarity with electronic medical record (EMR) systems
  • A collaborative mindset and enthusiasm for professional growth
